Yakima Valley Memorial Hospital began providing Chronic Disease Self-Management Program classes (CDSMP), called Taking Control of your Health (Tomando Control De Su Salud) in 2008. The program participants meet 2 ½ hours once a week for 6 weeks. Curriculum topics include: nutrition, exercise, goal setting, pain management, and self-management. Participants who took the class reported a 46% increase in how they exercise each week and a 25% improvement in daily fruit and vegetable consumption. The overall goal for Memorial’s CDSMP program is to build self-confidence in maintaining and managing one’s own health and chronic disease condition.
Classes are offered in both English and Spanish. The classes are offered free of charge thanks to grant funding received from a variety of funding partners.
